5/7/2023 EnchantaVaughn Approved 1.Behavior: Anthony greeted QMHP-C as QMHP-C arrived to the home. Anthony appeared to be in a positive mood while discussing with QMHP-C how his day was going. He reported that his day was going okay. Ms. Broadnax reported his behavior in the community and home setting such as him fighting the neighbor over a toy that was not his. 2. Behavior: Anthony became uncooperative at times within the session as he was throwing items at his brother as his mother was discussing his behavior about him fighting. He at times screamed to the top of his lungs as well as utilizing profanity. 1.Intervention: QMHP-C facilitated an anger management activity and engaged Anthony to assess progress with skill use. QMHP-C probed for reasons why he continues to struggle with respecting authority. QMHP-C assisted Anthony in identifying additional coping skills to utilize when feeling angry and frustrated with others. 2. Intervention: QMHP-C reviewed consequences for inappropriate behavior and addressed alternatives for acting out. QMHP-C challenged Anthony’s way of thinking throughout session. QMHP-C provided feedback as it related to anger management activity and his behavior as a result of his feelings of anger. QMHP-C provided role play activities during the session in an effort to promote understanding. 1.Response: Anthony verbalized difficulty with remembering to apply learned skills when he is really upset and attempted to blame others for his inability to get along well with others for more than a day at a time. With assistance from QMHP-C Anthony was able to identify behaviors that he repeats as well as identified specific situations that have in the past triggered his anger and how the anger affected his lack of relationships with others. 2.Response: Anthony admitted to QMHP-C that is his weakness and mostly the reason as to why he gets in trouble in the community and when he was in school because he doesn’t like people.
